Triage Specialist
To plan and manages the triage activities. Follow processes in accordance with organizational standards to ensure correct siting, accuracy and operational effectiveness.
MAJOR DUTIES AND RESPOBSIBILITIES
(A) Operational
- Obtains history, gathers triage information, assesses and evaluates patient health concerns made via phone, fax, email or other modes of communications to determine right siting of care and provide health advice according to established guidelines and protocols.
- Develops, implements, and evaluates a plan of care for each caller's presenting symptoms.
- Collects and analyzes data to establish health priorities and interventions.
- Streamline the triage process and shorten the waiting time for patients requiring Emergent care or Specialist consult.
- Maintains and safeguards confidentiality of patient health information and medical records.
- Communicates complex triage case with the relevant clinician.
- Network with the clinicians from clinical departments to establish and update clinical triage protocols.
- Provide daily updates and monthly reports of the Triage services.
- Provide regular in-house training and assist in preparing quiz for the department so as to improve the team's medical knowledge.
(B) General
- Perform daily housekeeping of workstation to maintain neatness of the workplace using 5S concept and ensure confidential information is safely kept at all times.
- Ensure that all tasks requiring follow-up are being carried out.
- Ensure teamwork and participate in any and all activities related to the smooth running of the department.
- Attend continuing education I in-house training on a regular basis to maintain his/her competency.
- Participate in quarterly internal department test I quiz I audits.
- Exercise cost saving and adopt environmental-friendly measures at all times.
- Identify areas of improvement or participate in improvement projects.
- Perform other duties as requested by Executive and/or Manager.
JOB REQUIREMENTS
(A) Education, Training and Experience
- Diploma I Degree holder with minimum 2 years clinical experience
- Experience in Emergency department, Ambulatory care or previous telephone consultation is an advantage
- Communicate in English and mother tongue language fluently. Able to speak dialect I third language is an advantage
- Proficient in computer navigation and administrative skills
(B) Professional License
- Registered Nurse with Singapore Nursing Board or equivalent is preferred
