Patient Care
1. Providing patient-centred care
1.1. Deliver evidence-based care in collaboration with inter-professional teams
1.2. Demonstrate clinical skills, knowledge and critical thinking in the delivery of 
patient care
1.3. Develop, implement and evaluate the nursing care of a group of patients
1.4. Facilitate admission, transfer and discharge of patients
1.5. Assess patients’ needs on admission and each shift throughout their 
hospitalisation
1.6. Plan and implement care according to patients’ needs
1.7. Participate actively in physicians’ rounds
1.8. Carry out treatment and investigations prescribed by physicians. Prepare and 
assist accordingly
1.9. Evaluate and monitor patients’ progress according to planned care
1.10. Document accurately and concisely the care provided and its outcome
1.11. Facilitate transition of care coordination within and between teams and across 
settings
1.12. Collaborate and co-operate with physicians and other health care providers in 
the smooth delivery of patient care and services
1.13. Monitor compliance of infection control and practices in the clinical area
2. Engage patient in management of health conditions
2.1. Provide individualised patient / family education on disease prevention, process 
and rehabilitation
2.2. Administer self-management plans to empower patients, family members and 
caregivers to facilitate participation in own health management
3. Provide emergency care
3.1. Participate in medical emergencies or crisis situations
3.2. Monitor for early signs and/or deterioration of patients’ conditions with 
appropriate physiological assessment and observation
4. Provide clinical supervision
4.1. Provide guidance, direction and teaching to the nursing students, new staff and 
juniors in order to ensure that quality care rendered is to all the patients
4.2. Provide guidance and orientation to nursing staff assigned to the ward / unit
4.3. Provide feedback on clinical performance of nursing students
4.4. Take on the role as a preceptor and precept new nurses in learning activities
5. Participate in quality and patient safety activities
5.1. Implement clinical risk and quality improvement initiatives activities
5.2. Monitor individual standard of practice with available benchmark data at 
department level
5.3. Report near miss and error using organisational accident reporting system
Management
6. Implement established operational and nursing policies and procedures.
7. Exercise economy and care in the use of equipment and supplies.
8. Participate in activities related to the smooth running of the ward / unit.
9. Maintain a clean and safe working environment.
10. Assume additional responsibilities as assigned by Nurse Clinician / Nurse Manager / 
Senior Nurse Manager / Senior Nurse Clinician.
