Staff Nurse, Community Mental Health Team (CMHT)

Responsibilities:

To provide home-based psychosocial rehabilitation, follow up on treatment and support to patients and their caregivers in the community. You are also required to collect data for monitoring and evaluating patients' progress over time.

Requirements:

• Registered Nurse with SNB
• Possess a Registerd Mental Health Nurse /Adv Dip Mental Health or other relevant qualifications (e.g. social work, counseling, psychology)
• Working experience in psychiatric setting (for those with RMN/Adv Dip mental Health) with at least minimum of 6 months experience in acute setting, or experience in psychiatric setting for at least 4 years with at least 6 months experience in acute setting
• Passion to work in the community
• Possess good interpersonal, communication, writing & IT skills
• Familiar with healthcare environment
• Able to work independently with good decision making skills
