Telehealth Nurse Positions - SSN1/SSN2

NHG Population Health is recruiting experienced Registered Nurses to join our expanding Telehealth Service. These positions offer the opportunity to contribute to Singapore's healthcare transformation through innovative virtual care delivery.

Role Overview:

As a Telehealth Nurse, you will support the planning, operationalisation, and resource management of NHG's Telehealth Nurse Service (TNS), delivering care to clients through comprehensive telehealth platforms while coordinating care across multiple healthcare settings and community

Key Responsibilities:

Clinical Care Delivery:

• Provide safe, person-centred care in accordance with TNS policies and evidence-based practices

• Conduct tele-triage to assess medical urgency and guide appropriate care decisions using protocolised guidelines

• Deliver care advisory for symptom management and health education to clients and families

• Support Enrolled residents and GPs with tele-healthcare and case management services

 

Care Coordination:

• Navigate client and family experience throughout their entire care journey

• Coordinate seamless transitions between hospital, community, and primary care settings

• Collaborate with interdisciplinary teams, primary care providers, and community partners to ensure continuity of care

• Facilitate referrals and handoffs to appropriate care providers based on client needs

 

Hospital-to-Home Services:

• Conduct post-discharge follow-up calls and care transitions

• Evaluate client health progress and facilitate community care coordination

• Provide health information on disease prevention, rehabilitation techniques, and treatment compliance

• Manage co-care arrangements with community and primary care partners

 

Service Development:

• Support TNS development initiatives including workflow harmonisation, clinical systems, and process redesign

• Participate in ongoing audit and improvement activities

• Contribute to outcome delivery and service evaluation

• Maintain accurate documentation and call encounter records across multiple platforms

 

Professional Development:

• Participate in continuing nursing education and evidence-based practice activities

• Mentor junior staff and support team development

 

Essential Requirements:

• Registered Nurse with Singapore Nursing Board

• Minimum 2 years direct patient care experience in clinical settings

• Community nursing or Emergency Department triage experience preferred

• Fluent in English and a second language (multilingual abilities advantageous)

• Strong IT proficiency with ability to navigate multiple healthcare systems simultaneously

• Experience with electronic medical records, telehealth platforms, and clinical information systems

• Comfort with multi-channel communication platforms (voice, email, video, chat) and ability to switch between systems efficiently
