Are you a compassionate and skilled nurse passionate about supporting residents with complex needs, including those requiring palliative care? In partnership with nursing homes, we are looking for dedicated Senior/Staff Nurses to join our Care@Nursing Home team, where you will play a vital role in delivering holistic, person-centered, and palliative care to our residents.
Key Responsibilities
- Perform comprehensive assessments and screenings of nursing home residents, including those with palliative care needs.
- Develop, review, and implement individualized care and treatment plans, collaborating with nursing home staff and multidisciplinary teams.
- Provide clinical guidance, coaching, and training to nursing home nurses, with a strong focus on palliative and symptom management (e.g., pain, breathlessness, nausea, comfort, and relief).
- Facilitate Advance Care Planning discussions with residents and their families, ensuring their wishes and preferences are respected.
- Act as a resource person for palliative care, supporting and empowering nursing home staff to deliver high-quality end-of-life care.
- Ensure accurate documentation and database tracking for resident care, including palliative care interventions and outcomes.
- Participate in quality improvement projects and foster a culture of continuous learning and clinical excellence.
- Maintain cordial working relationships with all levels of staff and external partners.
Requirements
- Registered Nurse with a valid Singapore Nursing Board (SNB) license.
- Preferably an Advanced Diploma in a clinical specialty or equivalent.
- At least 2–4 years’ experience as a Staff Nurse in a clinical setting.
- Experience or strong interest in palliative care is highly desirable.
- Strong clinical reasoning, critical thinking, and analytical skills.
- Excellent communication, interpersonal, and mentoring abilities.
- Ability to work independently and as part of a multidisciplinary team.
- Positive work and learning attitude.
- Willingness and ability to work beyond regular hours, including evenings, weekends or on-call shifts.
