Senior / Staff Nurse (Community Nursing) *Only Singaporeans*

Job Description:

• The role of community nurses are to handle cases on a more short term care basis (approx. 3 months) as their role is to ensure that the newly discharged acute patients and their next-of-kin (NOK) are prep and has enough support to be doing the care for the patients moving forth. Hence, the role includes a lot of case management where they assess cases and recommend the resources that may be required, and empower residents and caregivers with skills and knowledge to take care of the patients.
  
  WH community nurse also has a nurse post where we ask residents to come for health checks, for education to residents etc. They are also required to be on standby during weekends to handle Emergency Department (ED) cases to prevent re-admittance (may be required to administer antibiotics, or do blood taking).

Requirements:

• Only Singaporeans
• Registered with Singapore Nursing Board
• Possess Diploma and/or a recognised Bachelor Degree in Nursing
• Those with Relevant Specialty qualification or Clinical experience will have an advantage
• Possess good interpersonal and communication skills, with positive work attitude and a strong commitment towards service excellence
• Ability to work in a team and in a fast pace environment
• Willing to work on weekends and public holidays and perform on-call duties
