Job Responsibilities:

1. Primary Care Engagement & Partnership

  • Implement partnership strategies that build the network of primary care and community partners, to provide accessible, appropriate and affordable care in the community; 
  • Foster and maintain relationships with GPs to understand their strengths and needs, and collaborate to meet the health and care needs of residents in Central-North Singapore;
  • Connect resources with primary care doctors and work with stakeholders within and beyond NHG to build their capacities and capabilities to care for their patients and solve system and process problems;
  • Manage contact database and strategic alliance with Primary care partners to ensure continuity of partnerships and collaboration

 

2. Care Models & Service Planning

  • Support planning and implementation of collaboration programmes, including shared care programmes, to integrate care seamlessly between primary care and tertiary care to improve patient outcomes;
  • Support planning and improvement of access of services for primary care partners including direct access, shared and community care services;
  • Support planning and implementation quality improvement framework to support Healthier SG preventive care objectives with primary care partners;
  • Identify opportunities for capability development, facilitate the implementation and use of collaboration platforms for the sharing of learnings, best-practices and training.

 

3. Financial Management Support

  • Promote continuous process improvement/ review of the assigned programmes and projects, to meet the organisation’s objectives and patients' / community’s expectations.
  • Complete tasks as directed by the Supervisor/ Director.

 

Job Requirements:

  • Degree holder in any related discipline.
  • At least 2-3 years working experience in community care/ marketing/ business, preferably in healthcare setting.
  • Willing to travel to region-specific GP clinics
  • Willing to work on Saturdays for GP events (est. once 3 months)
  • Proficient in Microsoft Office (e.g. Word, Excel, PowerPoint)
  • Possess good organizational, interpersonal and communication skills
  • Familar with the concepts of budgeting, operations and administration
  • Able to work independently with minimal supervision
  • Meticulous and accurate, able to complete tasks within timelines
