JOB PURPOSE

To support the work of facilitating, enabling, and driving care integration across with focus on enabling the integration of care protocols and bundles that meets the objectives & outcomes required for our population.  The staff is expected to be a team player, working closely with an expanded cluster-wide team comprising of multi-disciplinary team members. This includes supporting the development and implementation of pilots/ programmes by working closely with the various stakeholders like NHG healthcare institutions clinicians, Clinical Steering Committees, and their workgroups

 

MAJOR DUTIES & RESPONSIBILITIES

 

  1. Provide secretariate and administrative support to NHG Integrated Care Review workgroups and Clinical committees, including the provision of secretariat support which will includes curating agenda and take minutes of the meetings.
  2. Assist in the Programme Management and monitoring support with dashboard visualization reporting to build data analytics capability in evaluating program effectiveness. Track and report on program progress, including key performance indicators and outcomes to ensure successful program execution.
  3. Supports the coordination and integration of new and existing clinical programmes into integrated care plans and services and standardized care protocols, with the intention to evolve into mainstream bundled care plans/models across NHG population health strategic priorities and River of Life framework.
  4. Collaborate and provide support for stakeholders in various Clinical Steering Committees’ and Workgroups in the development of strategic Population Health programmes roadmap so that integrative care work planning, prioritization and implementation timelines are enabled for success.
  5. Facilitate the ecosystem mapping to identify current services or gaps and support the framing, strategic planning, and resourcing of integrated care programmes in tandem with the respective Chairs of the clinical Steering Committees and Workgroups.
  6. Undertake any other tasks or ad-hoc duties as assigned by the Supervisor
