Senior / Executive (Program Coordinator), Clinical Operations(Medicine)
1. JOB PURPOSE
- The National Healthcare Group (NHG) DEFINITE (DiabEtic Foot IN prImary and TErtiary) Care Programme is a multi-disciplinary programme which spans across primary and tertiary healthcare to enhance care for patients with Diabetic Foot Ulcers (DFU) and help prevent DFU-related amputations.
- The role of the coordinator is to support the data collation and analysis for key programme indicators, and ensure continuity of care for patients across inpatient, outpatient and primary care settings.
2. MAJOR DUTIES & RESPONSIBILITIES`
Specific
- Perform assessment, documentation, monitor, intervention, review patient’s needs throughout the continuum of care between primary care and hospital, and post-discharge.
- Provide education/counselling to patients and/or caregivers.
- Provide DM foot-related health coaching, assistance with Diabetic Foot Screening, assisting Podiatry in DM foot care
- Conceptualise development, implementation, evaluation of new strategies, procedures, workflows or systems in collaboration with healthcare team to improve clinical service and care.
- Support quality improvement and academic research by working with fellow DFCs, Research Assistants and Clinicians to design, plan, collect and analyse data, monitor and coordinate related clinical research projects/ trials, consenting patients for enrolment into the studies and registry, and any other tasks as assigned.
- Provide secretariat and administrative support as assigned by supervisor(s). This may include minutes taking, preparing meeting slides, tracking matters arising, keeping records, budget submission for programme funding etc.
- Cross cover for other endocrinology department events
General
- Where directed by Reporting Officer, and/or Deputy Director / Director, to support in other Woodlands Health-related projects and tasks.
3. JOB REQUIREMENTS
Education / Training / Experience
- Degree/ Diploma in science or health related field.
- At least 2 years and 5 years of clinical / research experience in healthcare sector for Degree and Diploma holder, respectively.
- Not averse to review and manage diabetic foot wounds.
- Experience in supporting write-up of clinical trial protocols and/or DSRB applications is an advantage

Others
- Proficient in Microsoft Office applications (Microsoft Word, Excel, PowerPoint) is essential.
- Familiarity with healthcare systems, research statistics programs such as SPSS or GraphPad Prism and research registry systems such as REDCap will be an advantage.
