Job Description:
Join our dynamic healthcare team in providing specialized nursing care to elderly individuals with dementia and mental health needs in the community.
This role combines direct patient care with professional education, offering opportunities to both deliver care and build capacity among community partners.
Job Responsibilities:
Clinical Care & Case Management:
- Lead comprehensive care planning and delivery within a multidisciplinary team, focusing on elderly patients with dementia and mental health needs in the community settings
- Perform detailed psychogeriatric assessments through home visits, including to patients in nursing homes and care centres
- Manage complex cases involving both physical and mental health conditions
- Coordinate care with other care teams/stakeholders
- Coordinate care transitions
Training & Capacity Building:
- Develop and/or conduct structured training programmes for community partners and eldercare agencies on psychogeriatric care, mental health assessment, and behavioural management
- Design and/or deliver case-based learning sessions and workshops to enhance the capabilities of frontline care staff
- Facilitate regular case conferences to promote shared learning and best practices in elderly mental health care
- Provide clinical advice to community workers in community settings
Stakeholder Engagement:
- Build and maintain strong partnerships with healthcare providers, social service agencies, and community partners
- Serve as a clinical resource person for community partners
Quality & Standards:
- Maintain high standards of nursing documentation and contribute to quality improvement initiatives within IMH and the National Healthcare Group (NHG)
- Develop training materials and standard operating procedures for community partners
- Participate in evaluation of effectiveness of both clinical and training programmes and implement improvements
Requirements:

Educational and Professional Requirements
- Registered nurse with Singapore Nursing Board, holding a Diploma or Bachelor Degree in Nursing
- SSN equivalent with at least 1 year of clinical experience in either mental health or geriatric care
- Knowledge of mental health or gerontological nursing is required; post-basic certification in either field is advantageous
- Experience in developing and/or conducting training programs
- Strong presentation and facilitation skills
- Strong clinical reasoning and decision-making skills
- Proven ability to work autonomously in community settings
- Proven ability to work in a team and engage stakeholders
- Proficiency in common healthcare IT systems and documentation
- Ability to speak local languages/dialects is an advantage
