Patient Service Associate – TTSH@Home (2 Rotating Shift)

 

Job Summary: We are seeking a dedicated Patient Service Associate (PSA) to join our TTSH@Home Virtual Ward team. As a PSA, you will play a vital role in supporting patient care at home by coordinating services, managing patient data, and ensuring operational efficiency. Your responsibilities will directly impact patient outcomes and contribute to a seamless care experience.

 

Key Responsibilities:

  • Patient Enrolment and Care Coordination:

Guide patients and caregivers through the enrolment process, providing service details, advising on charges of service, and offering financial counselling as needed (direct patient fronting).

Coordinate and schedule care services to ensure patients receive timely and appropriate care.

 

  • Administrative and Logistical Support:

Accurately record charges for consumables, services, and treatments provided

Manage logistical needs for patients to continue care at home, including supplies and equipment.

Perform system administration for patient admissions, discharges, and transfers, and ensure accurate data entry and audit compliance.

 

  • Data Management and Reporting:

Support data collation related to patient progress, patient flow, and operational metrics and ensure all documentation and patient information is compliant with hospital policies, clinical protocols, and regulatory standards.

 

  • Service Quality and Improvement:

Demonstrate exemplary service quality, assist with service recovery when needed.

 

Job Requirements:

  • Qualities:

Passionate about improving patient outcomes and advocating for virtual care services.

Strong communication skills, teamwork, and the ability to work independently.

 

  • Education & Experience:

Diploma in any discipline; preferred fields

Candidates with 1 year experience, ideally in care coordination, case management, or public healthcare are preferred.

Proficiency with MS Office and various IT applications related to patient care.
