Medical Social Worker (2-year)

MAJOR DUTIES & RESPONSIBILITIES

 

Clinical

Casework / Psychosocial Care

  1. Provide holistic case management support and appropriate level of care across the continuum of care services within the campus and into the community through assessment, care planning, interventions and evaluation of care.
  2. Member of Quality improvement related projects that will improve patient’s outcome.
  3. Staff will need to meet department clinical indicators.

 

Group Work

  1. Assist in developing, conducting and facilitating group work sessions to establish social support systems and networks for target groups
  2. Engage individuals and/or target groups who may benefit from group work.

 

Community Work

  1. Network and collaborate with community partners to conduct needs assessment and implement programs to enhance the psychosocial wellbeing of individual patients and community

 

Programme Development and Implementation

  1. Participate, implement and assist in the development of programs to improve the quality of care and services.

 

Education / Clinical supervision

  1. Provide guidance, training and co-supervision to social work assistant, senior social work assistant and student medical social worker in their standards of practice delivery.
  2. Participate in developmental programs, learning opportunities, and supervision for personal and professional growth and development.
  3. Conduct psychoeducational talks to patients/public with guidance as needed.
  4. Assist to conduct professional workshops and seminars.
  5. Assist to organize, coordinate and provide continuing education within Woodlands Health.
  6. Assist to develop resource materials.

 

Research

  1. Apply and demonstrate evidence-based practice.
  2. Critically evaluate current research literature and share with others
  3. Participate and support research data collection/design/analysis
  4. Assist to develop abstracts/posters/oral presentations

 

Management and Leadership

  1. Assist to manage services and resources to achieve targets set by department/ Woodlands Health
  2. Assist in supporting department’s workgroup to deliver and improve service and care
  3. Participate in department projects

 

General

  1. To apply social work ethics and values, legislation and international conventions to practice
  2. To apply strategies and be instrumental to advocate for beneficiaries
  3. To understand socio-economic-political system within a multi-cultural context and relevant policies
  4. Actively involved in department and Woodlands Health’ initiatives
  5. Any other duties assigned by Supervisor/ Head of department

 

JOB REQUIREMENTS

 

Education / Training / Experience

  1. Degree/ Post Graduate Diploma in Social Work
  2. For fresh graduate: To have completed minimally 400 of supervised placement hours.
  3. For staff with experience: Preferably to be eligible to apply for accreditation with the Social Work Accreditation Board (SWAB) or is a Registered Social Worker/ Registered Social Worker (Provisional)

 

Attributes

  1. Proficient in the use of Microsoft office (Word, Excel, PowerPoint)
  2. Ability to speak dialects, or Malay/ Mandarin/Tamil would be preferred
  3. Good Communication and inter-personal skills
  4. Knowledge of the social service sector and relevant Public Policies
  5. Need to act with integrity and in accordance to the social work code of ethics
  6. Team-player
  7. Passion in working in the social service sector
