Job Purpose

This role will contribute to developing and delivering value-based care strategies and portfolio in support of NHG’s vision for population health across the spectrum of care.

This role works closely with NHG institutions to coordinate initiatives related to value-based care models within the organization. This role requires a strong understanding of healthcare operations, a focus on cost-effectiveness and resource optimization, and the ability to collaborate across institutions and other stakeholders to drive improvements in care delivery. The role will also work closely with clinical stakeholders, Ops, Value and Finance colleagues in NHG in ensuring that desired value-driven outcomes are scaled across NHG.

 

MAJOR DUTIES & RESPONSIBILITIES

  • Prepare materials and facilitate discussions and meetings with internal teams and external partners to drive alignment on care models that prioritize value-based care principles.
  • Collaborate closely with institutions to identify opportunities for improvement in care delivery and resource utilization.
  • Analyze data and performance metrics to track progress towards value-based care goals and identify areas for further enhancement
  • Work with the institutions to develop and implement strategies to optimize key resources, reduce costs, and improve the quality of care provided to patients.
  • Stay current on industry trends and best practices related to value-based care, and incorporate new ideas into the team's strategy and operations
  • Prepare regular quarterly reports, meeting papers, and presentation materials for senior management and MOH.
  • To undertake any other duties assigned by the Supervisor

 

 JOB REQUIREMENTS

  1. Education/Training/Experience

 

  • Minimally a Bachelor’s Degree, with at least 5 years of working experience in healthcare
  • Strong communication skills, with the ability to collaborate effectively with diverse stakeholders.
  • Proven ability to interact with and influence stakeholders across the care spectrum
