Health Coach, Clinical Operations (Integrated Care) (3 years contract until 31 Mar 2029)
- JOB PURPOSE
The Health Coach supports and empowers WH patients through personalised health coaching to help them adhere to their care and health plans, adopt healthier lifestyle practices, and effectively manage their chronic conditions. The role includes partnering with patients to follow through on care plans developed by WH doctors and nurses, with the goal of improving their overall health and quality of life.
- MAJOR DUTIES & RESPONSIBILITIES
Specific
- Engage, motivate, coach, and support patients using positive psychology techniques to encourage sustainable behavioural and lifestyle changes through telehealth or home visits.
- Provide basic education to patient and their families on chronic disease management and relevant health topics [on-the-job training would be provided].
- Perform screenings to identify higher risk patients who may require close follow-up.
- Address challenges faced by patients in adopting health-promoting behaviours and support them in following their care plan.
- Track, document, and monitor patient engagements, progress and health outcomes accurately.
- Coordinate appointment and reminders to support timely follow-up and continuity of care.
- Identify patients who need further education and escalate care gaps or issues to nurses as required.
- Participate in regular case discussions with nurses and/or doctors to ensure coordinated and comprehensive patient care.
General
- Perform any additional ad-hoc tasks or assignments as directed by the Reporting Officer (RO).
JOB REQUIREMENTS
Education / Training / Experience
- Diploma or relevant certifications in the social-health sector is required.
- Ability to speak a fluently in mother tongue language and/or dialect is required.
- Experience and knowledge in chronic disease management will be an advantage.
- Prior working experience with patients is preferred.

- This role includes:
- 2 weekday evenings (working till 7.30pm)
- 1 Saturday half-day
- Flexibility to work-from-home on Saturdays when there are no patient appointments
