Family Nexus Care Ambassador

  1. Patient Care

  • Engage and perform relevant screening on children and family identified by or referred to Family Nexus

  • Provide basic and personalized health information to client / families

  • Provide support to client / families in navigating social-health system by giving information and resources targeting to their needs

  • Collaborate with other external agencies and community partners on referrals for timely follow-up

  • Publicize mother-child dyad and MSF parenting programmes to children and families

  • Identify client / families in need of subsidised services and facilitate referral to financial, social and health services

  • Perform accurate documentation in the delivery of care in client / families records

  • Support group-based health promotion activities conducted both, in the clinic and community, together with institution partners 

  • Provide support for engagement activities at clinic with supervisor’s or Clinic Head’s approval

  • Track, recall, reschedule referrals for Family Nexus clients who defaulted children and women’s health related services and follow up according to established workflows

  • Coordinate the care for client / families with the team in Women and Children’s Services

  • Abide by the institution’s policies and procedures on patient care at all times

 

  1. Patient Safety and Quality Improvement

  • Adhere to the International Patient Safety Goals (IPSG) and infection control practices

  • Comply with guidelines and safe practices according to organisational Work Instructions and Policies

  • Maintain a clean and safe working environment

  • Report unsafe practices and incidences in accordance to NHGP quality standards

  • Participate in continuous quality improvement activities

  • Adhere to the Personal Data Protection Act

 

  1. Professional Development

  • Attend appropriate training courses to acquire new knowledge and skills

  • Assess own learning needs and undertake learning as appropriate

 

  1. Administrative

  • Follow-up on referrals to and from Family Nexus

  • Assist to track clinical indicators set for Family Nexus  

  • Conduct and analyse surveys gathered from users’ feedbacks

  • Assist supervisor to plan and support group activities conducted in clinic or community

  • Perform telephonic follow-up if required

 

SECONDARY RESPONSIBILITIES

  • Participate and support organisational projects and events

  • Assist in planning, organising and coordinating activities with supervisor

  • Provide incidental housekeeping duties including maintaining cleanliness of workplace and topping up stationery and other consumables

  • Perform any other duties as assigned by supervisor
