STATEMENT OF PURPOSE
Oversee the daily operations of the YCH Referrals and Admissions Office (RAO) as well as ensuring that the department’s policies and procedures are in compliance with the organisational standards.
Be an expert practitioner in his / her clinical discipline.
MAJOR DUTIES AND RESPONSIBILITIES
(A) SPECIFIC (80%)
1. Lead the RAO team to screen all referrals from KTPH as well as other hospitals for patients that require admission in Yishun Community Hospital (YCH).
2. Coordinate with YCH and KTPH Clinicians to review, assess, classify and prioritise all the referrals based on YCH admission guidelines, protocols and work instructions.
3. Coordinate with relevant Allied Health Professionals such as Therapists and Medical Social Workers to review cases for their opinions, if relevant and required.
4. Lead the RAO team to ensure all incoming referrals are screened in a timely manner, and to ensure that all accepted referrals have relevant and appropriate bed allocations given in a timely manner.
5. Ensure that patients accepted for admission to YCH are admitted to the correct discipline, ward and class in accordance to the admission guidelines and criteria.
6. Support KTPH and other organisations by addressing queries and providing guidance with regards to YCH referrals and YCH referral and/or admission processes.
7. Discuss complex referrals with relevant teams in YCH and/or KTPH, including YCH Senior Clinicians, RAO Senior Nurse Manager, Physiotherapist, Occupational Therapist, Medical Social Worker as well as Infection Control Nursing team, Wound Care Team and Dementia Care Team.
8. Establish and maintain good working relationships with Clinicians, Nurses and Allied Health Professionals of inpatient wards to discuss plan of care and goals of treatment before receiving patients.
9. Supervise the RAO Administrative Assistants (AA) in ensuring that all referrals are handled and accordance to the department’s policies and guidelines.
(B) GENERAL (20%)
1. Possesses adequate skill in Microsoft Excel to maintain accuracy of data and timely reporting of information or statistics to different stakeholders.
2. Proficient in Microsoft Word / Powerpoint to facilitate meetings within or representing RAO. Key responsibilities include preparation of meeting material as well as writing of meeting minutes. Possesses good written and spoken English.
3. Provide support to various YCH and/or KTPH work-improvement projects or relevant audit or relevant research-related activities that contribute towards improving patient care and/or operational processes in YCH.
4. Continuously seek opportunities to improve RAO workflows, processes or organisation to support improving patient care and/or processes in YCH.
5. Assist RAO Department Head to review policies, protocols and work instructions relevant to RAO.
6. Support internal YCH and/or KTPH in training and familiarisation for RAO guidelines, work instructions and policies to relevant departments and teams.
7. Assume additional responsibilities or tasks as assigned by Director of Medical Services or RAO Department Head.
