Responsibilities 

  • Evaluate, plan and implement care of patients with chronic kidney disease.
  • Conduct teaching and counseling for patients and their families / carers.
  • Coordinate patient care where required.
  • Collaborate and co-operate with physicians and other health-care providers in the delivery of patient care and services.
  • Maintain related database.
  • Provide continuous and relevant updates, and submission of reports to the Programme Manager and/or Renal Champion Doctor where required.
  • Maintain a clean and safe working environment
  • Assist to review and develop patient education materials in consultation with physicians or other health-care providers where required.
  • Assist to review and evaluate work processes in consultation with physicians or other health-care providers where required.
  • Precept newly joined coordinators in delivery of patient care where needed.
  • Participate in quality improvement activities.

 

Requirements

  • Bachelor Degree in Nursing or Bachelor Degree in Life Science, Medical Sciences and/or related medical fields
  • Proficient in Microsoft applications.
  • A team player with excellent interpersonal and communication skills.
