The Challenge

To be part of a dynamic digital mental health service piloting a novel model of care in psychiatry. The successful candidate will be involved in the following areas:

(a) Enrolment

· Review eligibility of persons referred into the service.

· Onboarding of eligible persons into the digital platform.

(b) Remote monitoring

· Monitor the health status of persons in the service remotely.

· Engagement with persons enrolled in the service

· Follow up with patients and escalate to the assigned healthcare worker if needed, as per stipulated care protocol.

· Ensure critical information is communicated within the team.

(c) Others

. General administrative support

The Requirements

Diploma in any discipline.
Prior experience in mental healthcare and/or digital technology will be an advantage.
