JOB SUMMARY

Renal Coordinator is to counsel patients who have been diagnosed with End Stage Renal Failure (ESRF) about the different types of treatment modalities that are available in Singapore. A renal coordinator has to give information pertaining to Haemodialysis (HD), Peritoneal Dialysis (PD), Transplant and Conservative Care Management. He/She has to provide financial details in regards to the different modalities and cost differences of VWOs (Volunteer Welfare Organizations) and private dialysis centres. He/She has to coordinate patient’s care upon discharge from hospital and to upkeep data on patient’s preferred treatment modalities and dialysis centres.

The incumbent is also responsible for the administration of the renal right siting which provides disease specific case management for patients with chronic kidney disease (CKD) and triaging of referrals from polyclinics. He/she also maintains the database for CKD patients and conducts advance care planning (ACP) for specific patients referred by the nephrologists. 

 

MAIN DUTIES AND RESPONSIBILITIES (80%)

SPECIFIC 

  • Schedule and manage appointments for dialysis counselling, Chronic Kidney Disease (CKD) Clinic, Peritoneal Dialysis (PD) assessments and surgeries based on urgency and availability
  • Coordinate care transitions including referrals to Ang Mo Kio Home Support Programme
  • Liaise with nephrologists regarding patients' treatment choices (dialysis modalities, transplant or conservative management)
  • Monitor CKD patient care through liaison with polyclinic physicians, tracking laboratory results and referring abnormal cases to Nephrologists
  • Assist in data entry, compilation and generation of monthly reports
  • Provide admin support for CKD, dialysis counselling and PD program.
  • Assist in monthly checks on patient subsidy level by verifying information from the National Means Testing System (NMTS) to ensure accuracy of financial records
  • Ensure complete and appropriate documentation is maintained throughout patient care

SECONDARY RESPONSIBILITIES AND DUTIES (20%)

 GENERAL

  • Demonstrate knowledge and ability to monitor and coordinate care of the patient with CKD on medications, complications and interventions.
  • Recognize potential problems and/or significant changes in the patient’s condition during follow-up so as to initiate earlier evaluation and management by other healthcare personnel.
  • Communicate with referring physicians, other healthcare providers and other organizations regarding the patient’s status and outcomes.
  • Collect and evaluate registry data for review by the Division.
  • Ability to work successfully with referring physicians, other healthcare professionals and proven experience in program development / project management by CKD.

OTHER DUTIES AND RESPONSIBILITIES

  • Attend training courses.
  • Prepare to undertake any suitable task as assigned by the Reporting Officer or Head of Department.

 

JOB REQUIREMENTS

(A) EDUCATION, TRAINING AND EXPERIENCE

  • A degree in social sciences, psychology, counselling, social work or nursing. 

(B) PROFESSIONAL LICENCE

  • Training will be provided on the job.

(C) EXPERIENCE

  • Fresh graduates are welcome to apply.
  • Some experience in the healthcare industry would be an advantage.
  • Ability to speak dialects would be an advantage.
  • Must be computer literate.
  • Possess good interpersonal and communication skill.
