JOB SUMMARY

The Programme Coordinator – Kidney Supportive Care (KSC) is responsible for developing, implementing, and managing a comprehensive supportive care programme for patients with advanced kidney disease. The role involves strategic planning, coordination of multidisciplinary teams, patient-centered care initiatives, and stakeholder engagement to enhance the quality of life for patients.

The Programme Coordinator will ensure the effective delivery of services, education, and policy development to optimize patient outcomes.

 

MAIN DUTIES AND RESPONSIBILITIES (80%)

Programme Development & Implementation

  • Design and implement a structured Kidney Supportive Care programme aligned with best practices and institutional guidelines.
  • Develop policies, protocols, and pathways to integrate supportive care into nephrology services.
  • Ensure compliance with regulatory and ethical standards.

Patient Care Coordination

  • Facilitate multidisciplinary team meetings, including nephrologists, palliative care specialists, nurses, dietitians, and allied health professionals.
  • Oversee case management to ensure comprehensive, individualized care planning for patients.
  • Monitor patient outcomes and make recommendations for service improvement.

Education & Training

  • Develop educational materials and training programmes for patients, and caregivers.
  • Organize workshops, seminars, and awareness campaigns on kidney supportive care.
  • Provide guidance on advance care planning and end-of-life discussions.

Stakeholder & Resource Management

  • Engage with internal and external stakeholders, including healthcare institutions, policymakers, and patient advocacy groups.
  • Monitor funding and resources to support programme sustainability.
  • Collaborate with research teams to evaluate programme effectiveness and contribute to publications.

Quality Improvement & Reporting

  • Monitor key performance indicators (KPIs) and quality metrics for programme evaluation.
  • Prepare reports and presentations for senior leadership and funding agencies.
  • Implement continuous quality improvement initiatives to enhance service delivery.

SECONDARY RESPONISBILITIES AND DUTIES (20%)

General

  • Demonstrate knowledge and ability to monitor and coordinate care of the patient with advanced kidney disease.
  • Communicate with referring physicians, other health care providers and other organizations regarding the patient’s status and outcome.
  • Ability to work successfully with physicians, other healthcare professionals and proven experience in program development/project management to Advanced Kidney Disease.

OTHER DUTIES AND RESPONSIBILITIES 

  • Attend training courses.
  • Prepare to undertake any suitable task as assigned by supervisor.

 

JOB REQUIREMENTS

Education

  • Bachelor Degree in Nursing, Healthcare Management, Public Health, or a related field.

Experience

  • Fresh graduates are welcome to apply
  • Some experience in the healthcare industry is an advantage
  • Proficiency in spoken and written English language
  • Ability to speak differnet languages/ dialect is an advantage
  • Computer literacy, with proficiency in Microsoft Office
  • Possess good interpersonal and communication skills
