The programme aims to provide comprehensive, coordinated cancer rehabilitation services with capabilities to manage the continuum of needs from prehabilitation, acute rehabilitation to survivorship, for patients across all cancer types. It seeks to enhance patients’ quality of life by preventing functional decline, improving recovery and supporting long-term survivorship. TTSH will establish a Multidisciplinary Cancer Rehabilitation Centre, modeled after the NCCS-SCS framework, to bring together specialists practicing in relative silos and  build team capabilities.   By integrating care across phases, the programme will deliver seamless care to patients and achieve sustainable rehabilitation services.

 

JOB SUMMARY
 

The Programme Coordinator will facilitate the implementation of programme and support the operations, with the leadership of the Program Clinical Leads in the coordination, implementation and evaluation works related to the programme.

 

MAIN DUTIES AND RESPONSIBILITIES

 

SPECIFIC

 

  1. Assist in the preparation and set up of the programme/framework.
  2. Screen referrals for program eligibility and direct referrals to service tiers that best match patients’ needs.
  3. Engage eligible patients to enrol into the program.
  4. Collate data on patient’s progress and apprise the referring clinician for follow up issues.
  5. Assist patients to navigate the programme and redirect patients to suitable service tiers should patients clinically deteriorate, which may lead to a mismatch between patient’s needs and service intervention.
  6. Ensure relationships with key agencies and community partners such as Day Rehabilitation Centers (DRCs) are maintained.
  7. Implement workflows to ensure seamless referral and operations.
  8. Assist in enhancement of service and quality of the programme and its component services.
  9. Support and ensure smooth implementation of the programme, coordinate project team discussions and stakeholders’ engagements.
  10. Act as the Point-Of-Contact and facilitate clarifications relating to the programme when required.
  11. Assist in tracking of programme indicators and support progress updates/reports to stakeholders.

 

GENERAL

 

  1. Assist in planning, implementation, and management of activities and projects that contribute towards service/quality/process improvement and patient care.

 

  1. Be a change agent for inculcation of mindset change for Hospital-wide/Departmental level initiatives, to establish a working environment of collegiality and effective teamwork.

 

  1. Any other matters as assigned by HOD/Supervisor and/or Program Clinical Leads.

 

JOB REQUIREMENTS

 

EDUCATION

 

Degree in Nursing, Allied Health or related discipline

 

EXPERIENCE

 

  1. Independent with good initiatives.
  2. Possess excellent interpersonal and communications skills.
  3. Meticulous and organized.
  4. Proficient in Microsoft Office applications (i.e. Word, Excel and PowerPoint).
  5. Relevant/related working experience in acute/ intermediate and long term healthcare sector is advantageous.
