Co-Ordinator
The Case Manager will aid the Psychiatry Team, which include clinical screening, complex case management, database management and discharge management.
The main duties and responsibilities are summarised as follows:
- Practice case management in line with policy and established criteria.
- Coordinate with a Multidisciplinary Team to expedite medically appropriate care
- Apply clinical expertise to resource utilization and discharge planning
- Assist the Department of Psychiatry to participate in the development of clinical pathways, workflow, protocol, and practice guidelines.
Case Management
- Screen and identify admissions and select cases for case management based on established criteria
- Manage selected cases in accordance with practices in care delivery and coordination of multi-disciplinary practices
- Assess, plan, coordinate, implement, monitor, and evaluate patient care from admission to discharge into the community
- Interacting with other clinical care partners to ensure transition of care for patients.
- Provide information on available community resources and link them to patient’s needs
- Monitor, track and record variances in standards of care, delays in treatments and discharge as planned
- Conduct house visits to patient and interact with patient’s caregiver/family members when needed.
Clinical Quality Management
- Perform necessary audits, quality assurance activities and projects as needed.
- Collect and collate data on case management variances to support remedial actions and implementation to address gaps.
Community Education
- Assist in the planning, coordination, and implementation of educational activities for community service partners and the public
Job Requirements:
- Bachelor’s degree from any discipline, preferably in Psychology, Social Work or Nursing.
- Experience in caring for patients is an added advantage.
- At least 2-4 years of industry experience in an acute public hospital or healthcare institution.
- Good collaborative skills and ability to work with multiple stakeholders (clinical healthcare professionals, finance etc.) across different domains.
- Organised, meticulous individual with good project management skills.
- Ability to multi-task and work under tight deadlines in a fast-paced environment.
- Dedicated and willing to go the extra mile to see things through.
- Excellent written, oral communication presentation skills (English).
- Proficiency in Microsoft Office (Word, Excel, Powerpoint).
