Co-Ordinator (Renal)
Renal Coordinator is to assess, monitor, discuss and follow up on management plans of Renal patients diagnosed with Chronic Kidney Disease (CKD), with the objective of retarding renal progression, in a collaborative effort with the primary healthcare sector and the Restructured Hospital Renal Departments.
When patients are subsequently diagnosed with End Stage Renal Disease (ESRD), Renal Coordinator is to prepare the patients for dialysis initiation by educating patients on the different types of treatment modalities available in Singapore. Renal Coordinator is to provide information pertaining to Haemodialysis (HD), Peritoneal Dialysis (PD), Transplant, and Conservative care management. Renal Coordinator is to stay up-to-date with the latest development and changes in the available treatment modalities, in order to provide patients with the most relevant and accurate treatment options. Financial information of the different treatment modalities, cost differences and funding mechanisms of the community dialysis resources will also be proficiently provided.
Renal Coordinator maintains data on patient’s preferred and changes in dialysis modalities. They also act as liaison between patient and Renal multi-disciplinary team during the period of impending dialysis preparation.
For patients who have opted for conservative management, Renal Coordinator will arrange to conduct Advance Care Planning (ACP) discussions with patients and their Next-of-Kins (NOKs). Clear and accurate documentation and communication to the relevant healthcare teams would be required to guide patients’ continuity of care intra and inter-department.
For patients who express interest in Renal transplantation, Renal Coordinator is to be the point-of-contact between the National Transplant Centre and Renal multi-disciplinary team, taking on the role to discuss eligibility, refer, monitor and follow up on patients’ transplant work-up and transplant outcomes.
Job Requirements
- Degree in social sciences, psychology, counselling, social work or nursing.
- Fresh graduates are welcome to apply and On-the-Job Training will be provided.
- Possess good interpersonal and communication skill.
