Job Description:

To provide essential case management services to individuals with mental health challenges. 

 

Job Responsibilities:

  1. Perform assessments to determine client needs, health status, and social conditions
  2. Develop individualized treatment plans, coordinating services, and offering ongoing support to facilitate client recovery
  3. Collaborate with clients, families, and multidisciplinary teams to develop intervention plans
  4. Respond to client crises by conducting risk assessments, safety planning, and arranging immediate care to stabilize the situation
  5. Contributing to quality management and general departmental activities

 

Requirements:

Educational and Professional Requirements

  1. Diploma/ Degree holder
  2. Nursing and allied health background with psychiatric clinical experience.
