Case Manager
Job Description:
To provide essential case management services to individuals with mental health challenges.
Job Responsibilities:
- Perform assessments to determine client needs, health status, and social conditions
- Develop individualized treatment plans, coordinating services, and offering ongoing support to facilitate client recovery
- Collaborate with clients, families, and multidisciplinary teams to develop intervention plans
- Respond to client crises by conducting risk assessments, safety planning, and arranging immediate care to stabilize the situation
- Contributing to quality management and general departmental activities
Requirements:
Educational and Professional Requirements
- Diploma/ Degree holder
- Nursing and allied health background with psychiatric clinical experience.
