Case Manager (EPIP)
The Challenges
To be responsible for the provision of case management services for individuals aged 13 – 40 years old, suffering from early psychosis or at risk of developing psychosis. The successful candidate will be involved in the following areas:
Clinical
- Provide and coordinate continuity of inpatient and outpatient care from initial consultation through to recovery
- Conduct assessment of patients for medical, psychological, social, environmental, vocational and financial needs
- Formulate and develop psychosocial intervention plan with patient and / or caregiver
- Implement intervention plan and monitor patient’s progress
- Review and evaluate effectiveness of intervention plan at appropriate juncture with patient and treating team
- Provide supportive therapy and psychoeducation to patient and caregiver
- Conduct risk assessment and management proficiently
- Work in close collaboration with multi-disciplinary team and community partners
- Conduct group work with patients and/or caregivers
Research
- Conduct research ratings with patients and/or caregivers
The Requirements
- Degree in Social Work or Psychology preferred
- Working experience with young persons, or individuals suffering from psychiatric disorders will be of added advantage
- Must be a team player with passion in mental health recovery, strong communication skills and willingness to grow professionally with the organisation
