STATEMENT OF PURPOSE
The Executive of Care Transition Services (CTS) will support the operations and management of care transition services and programme between the NHG hospitals and the community.
The Executive will support the satellite office in each hospital by working closely with the hospital specialists on right siting initiatives and to work with hospital operations to review discharge practices and work processes. This includes working with Care Coordinators to facilitate right siting of discharge patients appropriately and efficiently into primary care and community care sectors.
The Executive will also support in seamless bi-lateral flow of care between hospital and community by supporting Clinical Liaisons in the hospital to support the development of clinical pathways and operational processes for appropriate patient flow between the hospitals and community. The Executive will also coordinate with other units of Population Health Campus to link up resources in the community and primary care sector to the hospitals. |

MAJOR DUTIES AND RESPONSIBILITIES
Specific:
Service and Programme Support
- Operations administration for Programmes:
- Perform required administrative tasks to support assigned operating unit(s)/area(s), services, or programmes.
- Ensure smooth daily operations of the assigned operating units/areas, services, or programmes.
- Collaborate with clinical disciplines, operations and clinical teams to ensure discharge resources and care paths are communicated and roll out as planned.
- Ensure that relationships with care partners (internal and external) are maintained across the continuum of care.
- Track and monitor relevant datasets.
- Prepare and assist in analysis of reports.
- Assist in set up/ maintenance of discharge planning resources.
- Assist in drafting reports, papers and other documents as required.
- Project management:
- Support supervisor and Clinical Liaisons to initiate and manage projects relating to clinical pathways and operational process development/ enhancement.
- Ensure that planned projects are managed, communicated and rolled out accordingly to planned timeline and budget.
- Service and programme performance:
- Assist to ensure that volume and financial targets for the assigned operating unit(s)/area(s), services or programmes are achieved.
- Assist to ensure that the programme operates within budget allocated.
- Ensure that all service and operations standards and requirements are fulfilled.
- Update and maintain assigned components of the department’s tracking and monitoring systems.
- Assist in planning and implementation of changes required to improve service levels and raise quality standards.
