RESPONSIBILITIES

 

  • To work with internal & external stakeholders/partners to plan and implement palliative and end of life care programmes.
  • To work with clinical teams to provide secretariat and operational/project management support for various care workstreams (e.g. Frailty, Palliative Care, Preventive Care) or any project / initiatives (e.g. advance care planning, care transformation, etc.) under the Population Health Campus.
  • Establish and maintain sector-based partnerships for scaling up efforts of community-based initiatives across Central-North Singapore and to streamline inflow and outflow of patients from hospital to community.
  • To provide fund administrative support by managing financial resources, ensuring accurate allocation, tracking of funds across stakeholders and reporting of outcomes, in compliance with organisational policies and funding requirements.
  • Support the Manager in financial budgeting exercises and support timely monitoring of key performance indicates and budget/resource utilisation.
  • To lead/execute the development of enablers to drive efforts to improve capabilities of community care sector collaborators & share best practices and harness digital technologies or quality improvement methodologies to improve care integration.
  • To work with the Manager in monitoring identified social and health outcomes to inform service planning and performing periodic reviews of key indicators for programme evaluation and continuous improvement.
  • Conducts all activities with the highest standards of integrity, professionalism and confidentiality. Complies with all applicable laws, regulations, policies and procedures, supporting NHG’s corporate integrity efforts and to uphold NHG’s corporate reputation by acting in an ethical and appropriate manner, reporting known or suspected violations of applicable rules, and cooperating fully with all organisational investigations and proceedings.
  • Builds and maintains harmonious working relationships and good teamwork among colleagues and external stakeholders.
  • Provide secretariat support for Population Health Campus and Community Care related meetings / training sessions. Duties include (but not limited to) scheduling and coordinating meetings, preparing meeting materials, minutes taking, circulation of agendas/reports/approved minutes and follow through of agreed action items.
  • Oversee, supervise and appraise the performance of any subordinates and develop subordinates in the course of work.
  • Perform ad-hoc duties as and when assigned/required.

 

JOB REQUIREMENTS

 

Education / Training / Experience

  • Possess a recognised degree in any discipline with at least 3 - 5 years of working experience preferred.
  • Practical knowledge and experience in acute or intermediate and long-term care sectors will be an added advantage

  • Strong project management and administrative skills also required, together with, good critical thinking and problem solving abilities.
  • Enjoy working in a fast paced and dynamic environment both independently and as a team player, yet is organised and meticulous.
  • Proficient in full suite of Microsoft Office tools - Microsoft Excel, Word, Powerpoint, Outlook and Visio.
