Associate Executive
A. PRINCIPAL RESPONSIBILITIES
ADMIN DUTIES
- Support in Healthier SG-related work at general practitioners (GP) clinics;
- Liaise with the service providers in arranging annual eye and foot screening deployments for patients with diabetes in GP clinics and community nodes;
- Schedule patients’ appointments for ancillary services, calling of patients for appointment reminders and payment collection during deployments.
- Submit reconciliation report after each deployment.
- Work closely with GPs and with Clinic Assistants to ensure patients adhere to their scheduled follow-up visits, blood tests and screenings;
- Support administrative functions for the NHG chronic care pilot programme including patient enrollment, documentation management, and coordination with pilot sites;
- Maintain accurate records and databases related to chronic care pilot activities and patient participation.
CASE MANAGEMENT
- Partner GPs in social prescribing by connecting patients to community resources to improve their overall health and wellbeing. This includes referral of patients to community and social partners for various care services such as community groups, exercise programmes, support groups as well as day care or home care services;
- Support GPs in better integration and coordination of patient care with various community / social partners;
- Provide technical support to elderly or less tech-savvy patients in using HSG-related mobile applications such as HealthHub, Healthy 365 Apps.
- Work closely with the PCN nurse to identify patients who require nurse counselling, spirometry and other relevant services;
- Support patients in adhering to their care plans by providing simple lifestyle and behavioral modification advice.
DATA ANALYSIS AND EVALUATION SUPPORT
- Collect and compile data from chronic care pilot sites including patient outcomes, service utilisation, and programme metrics;
- Assist in data entry, validation, and cleaning to ensure accuracy and completeness of pilot evaluation datasets;
- Support preparation of regular monitoring reports and dashboards for chronic care pilot performance tracking;
- Collaborate with evaluation team to gather qualitative feedback from patients, healthcare providers, and community partners participating in the pilot;
- Assist in organising and analysing survey responses, interview transcripts, and focus group findings related to pilot evaluation;
- Support preparation of evaluation materials including data summaries, charts, and preliminary analysis for management review.
B. SECONDARY RESPONSIBILITIES
- Gather feedback on ground issues and provide inputs on potential area for improvement;
- Participate in quality improvement projects/activities;
- Support the admin team in collating data for analysis and reporting if needed;
- Contribute to evaluation activities and research initiatives related to chronic care delivery and population health outcomes;
- Perform any other duties assigned by the Reporting Officer.
