Job description: 

  1. ERAS (Enhanced Recovery After Surgery) Implementation and Care Coordination (45%)
    1. Implement and monitor ERAS protocols across designated surgical conditions
    2. Coordinate pre-operative assessments and patient education sessions and materials
    3. Provide pre-operative counselling and education to patients and families
    4. Provide post-operative discharge support services to patients within 30 days of discharge
    5. Monitor protocol compliance and document variances
    6. Coordinate care across multidisciplinary teams throughout the patient journey
    7. Track and follow up on patient outcomes and recovery metrics
    8. Facilitate communication amongst various healthcare providers

 

  1. Patient Care Management (25%)
    1. Conduct pre-operative screening and baseline assessments
    2. Provide patient education and counselling throughout the care journey
    3. Coordinate pre-habilitation programmes job description of the VBC CPEs when required
    4. Monitor post-operative recovery and coordinate follow-up care
    5. Facilitate discharge planning and transition of care
    6. Provide post-discharge support and follow-up (e.g. Home rehab and Home nursing services)

 

  1. Data Management and Quality Improvement (15%)
    1. Collect and maintain data for ERAS compliance and outcomes
    2. Track key performance indicators including length of stay and readmission rates
    3. Support the implementation of Patient Reported Outcome Measures (PROMs)
    4. Participate in quality improvement initiatives
    5. Generate regular reports on ERAS performance metrics
    6. Assist in analysis of outcomes data

 

  1. General duties (15%)
    1. Participate in departmental meetings and quality improvement projects
    2. Support the implementation of Value-Based Care initiatives
    3. Assist in developing and updating patient education materials
    4. Participate in staff training and education sessions
    5. Support any other Value-Based Care related work as requiredThis position will be deployed to one of the three NHG hospitals (Khoo Teck Puat Hospital, Tan Tock Seng Hospital, Woodlands Health).

Requirements:

  1. Diploma holder
  2. Prior working experience in Nursing/Clinical or patient care areas will be an added advantage
  3. Good computer literacy
  4. Good communication skills, with the ability to speak common dialects an added advantage
  5. Able to work meticulously, independently and be a team player.
