The Population Health Management (PHM) Directorate has been established to support daily clinical management to become more systematic and targeted; to ensure NHG is more informed by clinical, operational and demographic data as Regional Health Manager for Central and North Singapore; and that this informs the way in which we deploy our collective resources across all organisations to maximise the health outcomes of the local population, and establish system planning and care delivery so it is more coherent and systemic in its understanding of population health outcomes, value, and equity.

 

Our role in the PHM Directorate is to facilitate this change across NHG at population, provider, place, and resident level – ensuring that we address not just the clinical and preventative aspects of care but also the social, environmental, and behavioral determinants of health – so that we can achieve our ambition to add 15M health years for our residents.

 

This means that the Population Health Management and Value Directorate is principally an ‘enabler’ of this operational and cultural change – developing enabling infrastructure and building new capability at every level of our system in NHG on behalf of our partners and population.

 

Major Duties & Responsibilities

 

The Assistant Manager will support the Population Health Management (PHM) leadership in developing and maintaining comprehensive population health profiles, segment populations based on health data and risk factors, drive the population health research agenda, support the evaluation of population health programmes, provide analytical perspectives for the establishment of a more robust population health data infrastructure. Their contributions will be essential to improve care quality, outcomes, equity, and optimise system value for defined patient populations and NHG residents.

 

The post holder will support the team in the following areas:

  • Profiling and Segmentation: Drive population profiling and segmentation to inform system planning and care delivery to become more systematic and targeted, and
  • Population Health Research and Evaluation: Use advanced data analytics to generate actionable insights, enabling researchers to identify trends, evaluate interventions, and prioritize areas for population health improvement
  • Business Intelligence: Leverage data analytics to fulfill user requests and provide actionable insights, enabling stakeholders to address business needs and drive informed decision-making
  • Population Health Data Mart: Harness understanding of healthcare data systems and databases to guide the development of a population level data mart that facilitates efficient analysis and reporting for population health. 

 

Key areas of responsibility:

  • Leverage multiple data sources to collect, integrate, analyse, and visualise data to effectively profile and segment our resident population
  • Support the set-up of the population health research programme to develop a learning ecosystem translating population health research into practice
  • Guide the generation of population insights to identify gaps in prevention and care, and evaluate the impact of interventions and projects
  • Serve as at the lead analyst for the development of the Population Health Data Mart

 

Job Requirements

Education

  • Degree in Business Analytics, Data Science Information Technology, Mathematics, Statistics is preferred
  • Master’s Degree in Public Health (with a focus on Biostatistics), Data Science, or Analytics would be an advantage

Experience

  • Relevant experience in population health or healthcare setting is preferred
  • Expertise in the use of MS Excel (VBA/Macro)
  • Expertise with basic statistical analysis in a statistical package of candidate’s choice required
  •  Experience with Python, R, STATA, SAS, or EPIC will be an added advantage
  • Proficiency in developing data visualization, dashboards and reports using Oracle OBIEE/OAS or Tableau
  • Expertise in analysis and presentation of data required
  • Equipped with skillsets on SQL, database in Databricks, Tableau publishing and administration is preferred
  • Good understanding of data granularity, structure, and relationship between data tables
